Successful Hemospray treatment for recurrent diclofenac-induced severe diffuse lower gastrointestinal bleeding avoiding the need for colectomy.

چکیده
Endoscopic hemostasis is difficult to obtain in diffuse bleeding from broad lesions in the colon. Hemospray (Cook Medical, Limerick, Ireland) is a mineral-based granular powder that absorbs water and activates the clotting cascade [1]. It is approved for nonvariceal upper gastrointestinal bleeding [2] but, other than in Canada, it is not approved for use in the lower gastrointestinal tract. In the case reported here, Hemospray was used as a “last-resort” therapeutic option to avoid an emergency colectomy. A 66-year-old woman with no previous history of gastrointestinal bleeding presentedwith a severe episode of lower gastrointestinal bleeding (drop in hemoglobin from 13.1g/dL to 7.9g/dL) 6 days after total hip replacement. Her medication included nonsteroidal anti-inflammatory drugs (NSAIDs) in the form of diclofenac 50mg three times daily and a proton pump inhibitor (PPI). Colonoscopy revealed a blood-filled lower gastrointestinal tract with diffuse active bleeding in the right hemicolon and cecum from multiple broad, superficial ulcers (●" Fig.1a). The endoscopic appearance was compatible with ischemic or NSAID-associated colonopathy. Because there was a lack of reasonable alternative endoscopic therapies, we used Hemospray in the cecum and ascending colon with the aim of avoiding the need for the patient to undergo an emergency hemicolectomy. Bleeding was controlled immediately (●" Fig.1b). Recurrent colorectal bleeding occurred 2 days later. Repeat colonoscopy showed surprisingly well-healed fibrin-covered ulcers and mucosal inflammation in the cecum (●" Fig.1c) and ascending colon. However, diffuse active bleeding was found from multiple superficial ulcers in the transverse colon that had not been present 2 days earlier (●" Fig.1d). We decided to treat again with Hemospray.

Effectiveness of TC-325 (Hemospray) for treatment of diffuse or refractory upper gastrointestinal bleeding – a single center experience
Background and study aims TC-325 (Hemospray, Cook Medical) is a powder agent for endoscopic hemostasis in patients with upper gastrointestinal bleeding (UGIB). Although most publications are based on case-reports and retrospective studies, data on efficacy are promising. Here we report our experience with TC-325 for diffuse or refractory UGIB. Patients and methods Data on patients receiving...
متن کاملDiffuse idiopathic varices in the colon characterized by lower gastrointestinal bleeding.
Colon varices are very rare lesions that usually present as secondary to portal hypertension. Idiopathic colon varices are even rarer, and should be considered especially in the etiology of massive lower gastrointestinal bleeding. Forty-five year old male presented to our hospital with complaints of recurrent rectal bleeding for one week and severe anemia. The patient was transfused with 4 unit...
متن کاملReply to Xavier S and colleague: “Hemospray use in upper gastrointestinal bleeding from tumor – is it the answer?”
We appreciated the comments from Dr. Sofia Xavier and colleagues and read their case with great interest. This case is an elderly male patient who presented with acute recurrent tumor bleeding in the pre-pyloric area which was treated with adrenaline injection followed by bipolar coaptation 4 days prior to experiencing recurrent bleeding. Hemospray was applied to the blood clot on the lesion an...

A 78-year-old woman, on rivaroxaban medication because of atrial fibrillation, was referred to our department with upper gastrointestinal bleeding. Emergency esophagogastroduodenoscopy (EGD) revealed multiple duodenal diverticula with oozing bleeding from inside a large diverticulum near the papilla (●" Fig.1 and●" Fig.2). Because of the location, clips were not applicable, hence injection of e...
متن کاملThe efficacy of Hemospray in patients with upper gastrointestinal bleeding from tumor
BACKGROUND AND STUDY AIMS Currently, conventional endoscopic treatments provide an unsatisfactory hemostatic outcome in upper gastrointestinal bleeding from tumor. Hemospray has been shown to be useful in many active gastrointestinal bleeding conditions. This study aimed to compare the efficacy of Hemospray and conventional endoscopic hemostasis. PATIENTS AND METHODS Fourteen patients with ac...
